Here's how we do it.
We offer extra support beyond your doctor’s office—at home, online through a computer, tablet, or phone, or at our clinics in some areas. First, we take the time to learn about you and your needs through an initial visit. During this visit we will get a baseline understanding of your health and determine what kind of clinical and/or community-based support you might need.
We work together for you
They are and will continue to be your primary care provider (PCP).
They manage your overall healthcare.
They are informed of your medical care provided by Homeward providers creating a more connected experience for you.

They are an extension of your PCP outside their office walls, to provide additional care in your home or virtually.
They provide additional clinical support to you at home, in one of our clinics, or virtually.

Yes. Homeward works with your existing primary care doctors and specialists to help you meet your health goals. But care goes beyond a doctor’s visit, and we can also help with things like follow-up care, medication questions, insurance benefits, connecting with local services like meal delivery, organizing rides and household chores, and more.

No. Homeward works with your existing doctors and specialists to coordinate your care. If you don’t have a primary care doctor or are looking for a new doctor, Homeward can get you connected with one.
